Player Cards:
Every player in age divisions of U10 and older must have a current CISC player card to participate in soccer league play. The player card is used to verify the age of all participants, to ensure that they are included on the team roster and have signed a liability release for league play.
Cost: For new players, the photo card costs $10 and is good for one year. For players who have had a card in the past, cost is $5 to renew. If playing on a second team, a second card must be obtained at a cost of $5.
Expiration: Cards are good from September 1 thru August 31.
What is needed?: New players MUST provide proof of birth date (copy of birth certificate, passport, driver’s license, etc) and sign a CISC liability waiver. Players under the age of 18 must be accompanied by a parent/guardian. The application/liability release is provided below (page 2). Players renewing do not need proof of age but they do need to sign a liability release when renewing. This is a photo card so, obviously, the player must be present to get a card. We retake all youth photos each year.
Make sure you know: To obtain a card, the player must know the name of their team and what age division they are participating in.
When to get your card: Players can get their card at CISC anytime AFTER their team has registered. If you cannot get your card in advance and must get it the day of your first game we recommend that you arrive at least ½ hour prior to your game. There may be a waiting line.
If you forget to get a card: If you are a new player and forget to get a card or forget to bring proof of age, DO NOT EXPECT TO PLAY.
If you forget to bring/lose your card: CISC can provide you with a temporary card for one game at a cost of $1. You will not be permitted to play without it. If you lose your card, you can get a replacement at a cost of $5.
What to do with the card?: The card must be given to the referee prior to your game. The referee will return the card to you after the game unless a red card is involved. If a red card is issued, the referee will turn your player card in to the main office. You can pick up the card after any possible suspension is served.
Playing more than one session: If your team is playing in more than one session, you do not need to obtain a new card for each session. However, if you change teams, you must turn in your old card in order to get a card for the new team at no cost. If you do not turn in your old card you will be charged $10 for the new team card.
